As I pointed out on Twitter, this also works on recent bleeding-jumbo:

--format='dynamic=md5($u.$c1.$p),c1=:ManagementRealm:'

without requiring config file edits.

Dhiru suggests using a mix of your posting and the above command-line
dynamic format for a piece of documentation:

Jim - maybe you need to document the command-line dynamic format support
in some prominent place, so that more people are aware of it?
