Accomplishments:
- mscash-opencl supports all modes and much less prone to problems.
- mask initialization can be done in init(), reset() or anywhere. This adds
more flexibility and more cleaner codes. By default mask initialization is
done fmt_init() in format.c.

Priorities:
- Find more bugs and try to fix them. We already have some bugs that needs
to be fixed as discussed by magnum.
- Start working on LM format.
